*All waivers need to be filled out and emailed at least 24hrs before the presentation*
Required Waiver
To download the waiver please do the following
Right click on wavier image
Select Save Image As
On the left side of the new window select Desktop
In the bottom right of the window select Save
The wavier will now be on your desktop as an Icon
Waiver must be signed and delivered 24 hours before Apointment
